Describe What Happened: Contractor was entering sawmill thru designated walkway and was struck by forklift in the mobile traffic area. His L. foot was dragged under the L. tire of the skidding forklift. He was held upright in the boom of the F/L until it came to a stop.
Investigation Findings: Contractor was not looking towards oncoming traffic; visibility was restricted at the walkway: walkway was nor painted on roadway. There was no written policy or signage on mobile traffic right of way. Orange cones were used to identify the walkway
Remedial Actions: Develop a policy for mobile traffic on the worksite and orient the contractors of this policy and workers make walkways have a good field of vision at entrances paint all designated walkways and have signs identifying the right of way policy. Use of cones at walkways should be reviewed to find a better identifiers of the hazard.
Critical Learnings: Although workers are aware of the mobile hazards at there workplace contractors may not be aware and should be advised of them regularly and that it be documented that they have been.
Additional Comments: We wish the contractor a speedy recovery as this could have easily been a fatality.
